You get this for completing a challenge room. There's one located in each level, you enter them by interacting with a small square machine. Once inside a challenge will appear at the top of the screen like no jumping or don't get hit, if you complete the challenge you'll get an item and the achievement.
The no jumping one is very hard to do without the crocodile or rat skull, rat is better tho cus you can use it to warp straight up if you make a mistake.
There's also some challenge rooms that give you a skull at the beginning and you have to reach the exit, usually by making use of the skull you just received, that's how I learned that you could warp through walls with the rat skull (so OP).
