The achievement descriptions for this game are quite cryptic.
This achievement is for discovering the secret ending.
IMPORTANT:
Completing this achievement will reset your save data. If you haven't already found 100 worshipers or unlocked all of the other achievements (Chapel arena, etc.), you may want to do that first so you don't have to start all over.
To get the secret ending, you must first beat the final boss (no sweat, right?).
Once you beat the boss, you will continue in the sky and see the room in which your player is occupying.
(If you haven't already done so, you can access 'The Room' by pausing the game and selecting the 'tool' looking icon in the menu).
Anyways, you will see a four digit number on the wall in the back of the room. Make note of this number and enter 'The Room' from the menu.
One of the interactive objects in the room is a lock box. Use the four digit code you saw after the final boss fight to open the box and get the ending (and achievement).
