Achievement is unlocked after unlocking all 23 nodes in your mind map. As long as you talk with everybody and choose every dialogue option, you shouldn't have too hard of time with this. You don't have to have every entry, but you need to have the following 23 nodes.
Below is where you unlock the node for the very first time.
ACT 1
Kay/You - Available from the Start
Blake - Open the mind map when first available in your bedroom, and choose every dialogue option on Kay/You Node
Catherine - Open the mind map when first available in your bedroom, and choose every dialogue option on Kay/You Node
Ditch Man - Open the mind map when first available in your bedroom, and choose every dialogue option on Kay/You Node
Blue - Look at the Photo Album on coffee table in the Living Room
King Pierre - Look at the Photo Album on coffee table in the Living Room
Duck - Look at the Photo Album on coffee table in the Living Room
Shield - Look at Refinery from Backyard
Million - Talk to Million in Backyard
Client - After Talking with Million for the first time, open your mind map, select Catherine/Mom, and choose "Was conducting research..., For who?"
Troy - Talk to Troy at Dimes Discount on your first visit
Reports of Construction - Talk with Million on the way to Sarpy Paperbacks after purchasing fuse.
Something in the Lake - After talking with Million on the ride to Sarpy's, Choose Catharine in Mind Map and Select "Your Mom..., Was Conducting Research..., In the Lake"
ACT 2
LeBlanc - Talk with LeBlanc in Floodgate Tavern at the Start of ACT 2
Lucky- TV in Floodgate Tavern, interact with it.
Bloodline - Buy LeBlanc his first beer and then talk with him and choose all options
Laura St. Clair - After buying LeBlanc's second beer, talk with him and choose all options
Thomas Sinclair - After buying LeBlanc's second beer, Talk with LeBlanc about Laura, and ask "Who is her Father?"
Dimes- After Buy LeBlanc two beers and a meal, Select Duck from your Mind Map and choose every option.
Head Drive - Talk with Duck in his house and ask about "Head drive...?"
Fake Ring - Talk with Duck in his house and ask "Did you attend her Funeral?" followed by "The ring she wore around her neck was fake?"
Garrett - After entering Shield HQ, talk with the Balcony Crown in the lobby
ACT 3
Sunken Head - Read the Note on the desk, when you return to Duck's House

@suen szn: no, unfortunately you have to replay the whole game. That's what I had to do for 4 achievements I previously missed. It took me significantly less time in my 2nd playthrough though and I didn't have to play until the end, only the lake section, where one of the achievements I had missed was. Your mind map should have all the main entries before that, so if that's the only achievement you're missing, you'd have to only play until the beginning of act 3 when you return to Duck.
