This is missable but it's hard to. When going through the swamp in Act 3, you'll head over a place you can dive at near the last objective. You can keep going past it so listen for the tone. Dive in and you'll pick up Monkey and the achievement will pop.

Just to add this is after you pass the "Duck Blinds" landmark, having visited both red crosses on the map.

Also, you MUST pick up Monkey in Act 1, as it must fly off the truck when traveling for it to end up in the swamp.
