Edit: so i'm not sure if this is the fix, but i quit the game. Started new and then did like the video, quickly feed and leave the stairs so the letter/save icon was near the drink stations. Not sure if its a bug or just dumb luck. The other 2 times i was looking at "Mother" and waiting for the cheevo to unlock...

In the main starting place of the game, go forward towards the tower. Or, from the previous trophy (Special pills), go down the stairs and take a right. The door will say 'Enter Work'. Go forward until you see a pool of water with an alien in it. Interact with the alien to get the trophy.

This is unlocked after obtaining “Listener.”
On Level 1, after leaving HOME, stay to the right, and almost immediately turn right and walk straight towards a glowing door. This is the WORK door.
Enter and walk straight and eventually on the right you will see a mechanism with a figure taking a bath in some blue liquid. Press 
to ‘Feed’ it and will discover you have nothing to offer. As you’re walking away the achievement will unlock.
The next achievement you should unlock is “Burned out at work.”
