Every level has a green tube that gives you rad when it gets broken. Skip breaking it on a level, and skip breaking it again on the next level. The level after will spawn horror, a green blob looking guy who shoots a laser type beam at you. Just kill him to unlock Horror.
FYI the tube can be broken by bumping into it, and the portal that appears when you kill the last enemy will break it as well if it appears too close.

If you exit a level without destroying the radiation canister (green tube), the next level spawns with a bigger one. If you exit that level without destroying the large radiation canister, Horror spawns in the next level. Kill horror to unlock it.
