Here is my 100% Achievement Guide for Observation With ALL Collectibles!
At 4 points during the game, you will hear this electric, intense noise and a hexagon appears.
This hexagon then gives you 3 patterns which you then have to enter correctly. Kind of like a memory game.
To get this achievement, you have to complete all 4 of these WITHOUT a mistake.
Luckily, its so much easier than it sounds as...
1. If you do make a mistake, apparently replaying the same point using the
button does not void the achievement.
2. It is literally not random at all, so you can copy exactly what i have put down and it should always be the same.
There are 8 answers you can choose from, and to answer you have to click the obvious correct pattern by using any of the D-pad or the A,Y,X, & B buttons, depending on what the answer is of course.
If you want to see all 4 being complete, go to :-
1. 36:20
2. 1:21:30
3. 2:34:45
4. 3:08:20
So just copy excactly what i do and you will unlock this basically at the of the game.

I made a mistake on the 3rd puzzle, pressed LB to restart the puzzle and never got the achievement. So I'll have to replay the whole game now. 😩. My advice. If you make a mistake, dashboard the game and replace the latest section to be safe.

Same thing happened to me. In the third Hexagon encounter they have two symbols very similar (the pyramid of boxes) and I redid the first sequence as the guide suggested and didn't get achievement. So I also recommend going to dashboard instead.

This achievement is awarded for correctly repeating the symbols to the Hexagon entity each time it appears throughout the game. The Hexagon appears 4 times and below, is a video showing a quick walkthrough of the game and the time-points that the entity appears. Credit to codedot:

PLEASE NOTE: I think "replaying" the set of symbols with will void the trophy so I advise you don't use that at all and if for some reason you make a mistake, just quit to the menu to restart from the last save.

Thanks for the guide. I can confirm: quitting after messing one up and restarting will still get you the achievement

Replaying does not appear to void the trophy, as I sat down and replayed multiple during a single session playthrough and still got it. It seems the only requirement for it is to complete them all without failing.

If you lack the achievement of anomaly, like me, you can make a second pass with this speedrun, with doing 3 anomalies the achievement jumps, it is not necessary to do the 4

Can confirm this works. One word of advice; when the speedrunner quits out and saves, do so, or you'll have the anomaly to do that isn't necassary.

Type characters carefully and without errors. In the last section of the puzzle there are two types of “pyramids” and do not confuse them! If you typed correctly, there will be a blue blink that says >>>. If you made a mistake, there will be a red flicker and the inscription <<<. If you make a mistake, then repeat the entire puzzle from the last checkpoint. Video guide (en).
