There are four recordings found throughout the game.
1: By the third floor entrance, inside the cables.
2: Go past room 110 and check your left for a bathroom. Unclug the toilet and you will see the recording.
3: It is In the basement, in Room 020.
4: Near the end of the game, check for a sign labeled “Nanophage Infected Area” in the Sanctuary. Look on top of the table for the final recording.
Once the final recording has played, the achievement will unlock.
