Apparently my first rose that I picked up didn't count because my character didn't comment on it.

Maybe that's what happened to me too. Must need to have the first 3 register for the grave to show up. I'll do a speed run at some point to check if that's the case.

No gravestone for me. Tried restarting from last checkpoint and it's not there. Weird since even the walkthrough on here says it should be.

Do they have to be scanned to count? Does this have to be done in a single run or over multiple playthroughs?

The first rose can be found on the bottom floor of the apartment building on top of the pipes next to the stairs.

The second rose can be found on the second floor in the "maintenance access" near apartment 208.

The third rose can be found while following Victor's trail and going up the stairs to the bird coops.

After you have found the roses you can proceed in game till you get to the forest in which you can find a grave for the fourth and final rose.
