This is found in the Shrine of the Prince of Thieves, located directly below the town of Quarrycrest, to the West of Bolderfall. The area is danger level 20, so you'll need to be appropriately levelled to reach it, but there's nothing to the shrine - just walk in the entrance and there's an altar right in front of you to interact with.
