Super simple, you can get this in your very first battle.
Every turn you take builds up Bonus Points (BP) at the top right of the screen. Keep using your normal attack until you have at least 3 BP at the top right. Once you've accumulated enough, on your next turn press
three times until your attack boost is the highest it can go, then attack.

A Level 4 boost is attained by pressing 
three times to max out the effectiveness of an attack. You must either wait three turns in battle without spending any battle points (BP) or use items/skills to earn extra BP which you can then spend. Either way, unleashing a max skill will happen often, so this will come naturally.
