The earliest that this can be earned is on level 4 The Funicular
During this level you will learn how to craft an Antidote and unlock:
The Teach A Mud To Fish achievement in Oddworld: Soulstorm Enhanced Edition worth 21 pointsCraft your first recipe
The level contains 24 Mudokons, all of who need healing. Searching out the secret areas to find these Mudokons will net you this achievement during the level. (I think the level only needs you to heal 9 to complete it)
It may be possible to find a good checkpoint and reload it to speed this up, but I didn't test this as: 1. you will want to save as many Mudokons for the good level (and it's not difficult to find them) 2. You are going to need multiple playthroughs of the level for other achievements anyway!
Level 5 also contains 4 sick Mudokons early on, if you did miss any on level 4
If anyone confirms reloading checkpoint I will add to the solution, giving you credit
