This requires you to die by falling from a great height. I got it by falling from the very top of the cliff during the Ip’Tak boss fight on the Filap Islands near the end of the game.

If you've already defeated this boss, you can return to fight him again. Ascend until you reach a ladder on the right, where the tower opens. Climb up and jump out.

This is a bit of a weird trophy. I unlocked this by leaping to my death just to the right of the flute player in Filap Islands, but after the game was released, an invisible wall was apparently patched into the game (this is confirmed to no longer work for Steam players, but I'm not sure if consoles were effected.) If this spot doesn't work for you, here's a video showing an alternate method that works post-patch:

Elevators exist throughout the Olija game world. Activate one and immediately jump off, then wait for it to reach the bottom. Finally, jump down the elevator shaft to your death.
