How to unlock the It's all a dream... achievement in OMORI - Definitive Guide
✎ DETAILS AND RESOURCES
✼ The badge for naming your character OMOCAT after the prologue ends has been removed from the Xbox version. It's currently not known if it's still required for 100%.
✼ Any badge marked with a ☒ symbol is missable if you go past the point where it originates, but progress too much outside the window of opportunity.
✼ Any badge marked in bold is exclusive to the Normal playthrough. Proceed with caution.
✼ Badge order is listed as shown in the Xbox version. This should help clarify anything you're missing without much hassle.
✼ The Neoseeker walkthrough will help get you through the Normal and Hikikomori routes, while covering most of the badges. However, it is missing some details in the Normal route required for achievements/badges. I highly suggest using the Flowers and CD guide as a supplement to the Normal playthrough when you're working on optional content.
✼ To avoid cluttering this solution, I will only be providing small details to badges that are missable and/or vague. For the rest, please refer to the Neoseeker guide and this achievement guide.
✼ I strongly suggest saving most of the Headspace related badges/achievements for a Hikikomori playthrough (unless otherwise stated), as you're able to explore everything after One Day Left.
✼ Make multiple saves at all times, as it's possible to miss something by accident during the Normal playthrough.
You're a lot stronger than you think.

I can't argue with that complaint, honestly. The badges are mostly obscure, and I highly doubt anyone could figure them all out on a first, second or even fifth playthrough. Guides are absolutely essential if you want to finish everything within two playthroughs.
I personally knew what to expect after doing research before the Xbox release, but I will admit it's quite overwhelming making a path that ensures you don't miss anything. Sweetheart's movie theater was definitely one of the few low points (in a sea of many highs), but thankfully it's not difficult, just tedious.

My 82nd badge was 'MUSIC CONNOISSEUR OF SORTS'. I loaded all the CDs into the jukebox, popped that achievement, listened to some music, exited to the title screen, confirmed I had that badge and the 'IT'S ALL A DREAM...' badge and the count was "BADGE COMPLETION: 83 of 83 (100%)" but the 'It's all a dream...' achievement didn't pop. Exited the game, restarted the Xbox, took a late save position to the actual game end, watched the credits, still nothing.
UPDATE: Almost 3 weeks later and it still hasn't popped. Uninstalled/reinstalled and even started a new game and did a couple of actions that would have counted as badges but still nothing.
UPDATE: 25 days later and no unlock. Spent 2 hours with MS Support and, surprise, they had nothing after exhausting their script of tech support for morons.
UPDATE: The only solution was to delete all save data and restart from square one. 60 game play hours later and I got it. Absolutely unacceptable.

This achievement requires you to get all badges, which were originally achievements on the PC version. This requires the completion of both True and Hikikomori Route.
The [TR] means True Route and [HR] means Hikikomori route.
Excluding story-based badges and achievements on this list, here are the ones you need:
Green Thumb: water one of Basil's plants that have dried up. This is mandatory for the Secret Ending.
So majestic...so beautiful: This requires you to get the Butt Certificate before you enter Otherworld for the first time, so Kel can plant it as a flag. To get the certificate, complete Berly's quest to find a lost tetherball. As a reward, you get the certificate that gives Aubrey the ability to headbutt enemies.
WHAT'S THE BIG IDEA!?!?: In the Otherworld Campsite, you find a road covered in traffic cones. As Omori, cut them down until you reach the Lone Mole. He will lash out at you and force you to put the traffic cones back.
xD: You have to get the LOL Sword, by completing the Weeping Willow's side quest. You meet her in Pyrefly Forest, and by putting 10 clams into a sparkling lake and tagging Hero (since he's the only one confident enough), you can tell her a joke. Near her spot, you can find a Joke Book. To complete the quest, you need to make her laugh by finding a perfect joke. The only one that makes her laugh is the Construction Joke found in Deep Well (Underwater Highway), in a construction zone, which can be found above the second Toll Gator that forces you to pay.
Christmas Crusher: In Sprout Mole Village, you can find a Christmas Tree to the right. As Omori, cut it down. Great, now the Sprout Moles can't celebrate.
Spring, Summer, Fall and Winter Sympathizer: I categorize all of these as the same since they require the same thing. In Sprout Mole Village, if you go down and left, you can find four Sprout Moles, who ask you what season you like the most. Before this, save at Mari's Picnic. Choose one, go back to Mari, load the save, and repeat but with a different season.
AWEKJRLKJFLKASNFAWIJGAWEFJAWEKFJAKFAASJFKA!: This requires you to fail to bake a cake the first time during the cooking sidequest in Sweetheart's Castle, by making at least one mistake. You likely got this achievement accidentally, since the sous-chef's instructions are unclear or just flat out wrong.
Everyone's a critic: This is the most annoying badge to get, because it involves around RNG. But thankfully, you don't have to watch every movie, you can just skip them immediately when the first scene plays. You have to go to a movie theater in Sweetheart's Castle, and watch 11 movies. Problem is, they're randomized, so if you're unlucky, you will be at this badge for hours. The movies show one looped scene, such as: Sweetheart kissing a Sprout Mole, Sweetheart's dress being advertised, Sweetheart at the beach with a giant hole in her stomach, a badly drawn Sweetheart jumping from a dragon, a badly drawn Sweetheart dancing to weird music, an advertisement for Sprout Mole tofu, Sweetheart driving a car while a sunset is happening, a Sprout Mole who gets more friends, Sweetheart doing an illusion, a Christmas movie and a tennis match between two Sprout Moles
As expected from professionals!: During the choir mini-game, try to be as perfect as possible, so no Sprout Mole is asleep too long.
Hope and Vigor!: Complete the Conviction of Orange Joe quest in Orange Oasis, which has you travel to Dino's Dig to Oragne Joe.
The Chosen One: Complete the Rain Town quest, which has you interact with a pond in Orange Oasis and turning a valve. You should turn the valve right, since turning the valve left fails the side quest and you can never get back into Rain Town then.
Ain't nobody here but us chickens: Defeat the Chicken? on the fourth floor of Dino's Dig. Defeat it as quickly as possible since it will flee away and never respawn.
The currency of the future...: After getting your friends out of Last Resort, you will find Shady Mole near the Taxi. Interacting with him gives you the Clems sidequest, where you give all of your Clams for Clems, that are actually useless. To find him again, go to the Construction Zone where you can find the Construction Joke and go to the complete right, finding a Pearl Cave. He's much stronger than any other Sprout Mole and he uses Dynamite, which can deal significant damage to your party. After defeating him, don't forget to pick up your Clams.
Good Dog?: Find Marina's Sector in Humphrey and pet every creation. Poking one of them will make them disappear until you re-enter.
Mmm...SWEETHEART, I mean, TOFU: Self-explanatory, hold 99 Tofu at the same time. The easiest way is to buy 99 tofu from Smuggler Jash in Sweetheart's Basement. It will only cost 198 clams for 99 tofu, or 2 clams for 1 tofu.
Squizzard Exterminator: Similar to Bunny Exterminator, you have to defeat 100 Squizzards in the Seacow Farm, found in Underwater Highway.
A Bit Less Lonely: [HR] During One Day Left, pick up Mr. Jawsum's check at Last Resort and commission the Gator Guy on the 4th floor all of the statues of yourself and your friends. Thr quickest way for him to finish is exiting Last Resort and re-entering the 4th floor.
Take Me to the River!: [HR] You have to charge the Plastic Fish with Batteries and let it finish it's song. You can find the fish in Frozen Lake inside of an igloo. You can get batteries from Pinkbeard, who only appears on One Day Left inside of Space Boyfriend's House. When you get thid achievement, you also get access to Snowglobe Mountain, one of the exclusive areas to the Hikikomori route.
One for the Road: You have to get bread from Bread Slice. The easiest way to do this is get to Vast Forest, find a Sprout Mole or bunny, and slash them with Bread Slice. This is almost always a one-shot.
Seriously, you're the coolest!: [HR] This requires you to beat the Boss Rush, which you can activate inside of Slime Girls Lair when Humphrey resurfaces during One Day Left. Boss Rush is just defeating every boss in the game in a row
Up high...: [TR] High-five Kel. Just high-five him the first moment you get.
Down low....: [TR] High-five Kel three times. High-five Kel in all of the moments you can.
Too slow..: [TR] Don't high-five Kel. Also pretty self-explanatory, but I hope you got this only for the badge, because I don't like leaving him hanging.
Minty Fresh: Every time you wake up in the Real World, go to the bathroom and brush your teeth. Do this in Three, Two and One Day Left.
I'll just take that...: [TR] When you visit Kel's House during Two Days Left, go to his room and interact with the wardrobe. Pick "Yes" when you are given the option to take 20 dollars from him. This is payback for not giving me the money on time, Kel!
It's honest work: [TR] Complete all part-time jobs once. This involves Othermart, Fix-It and Gino's Pizza part-time jobs.
Against All Odds: [TR] Get a perfect score while delivering Pizza. Since the notes you are given have awful handwriting, just look at the Omori Wiki for the Pizza Delivery job, and you'll understand which houses you need.
That can't be good for business: During a part-time job, leave. If it's Fix-It or Othermart, interact with the Job Poster again to leave, and if it's Gino's Pizza, interact with the bike.
Good Company: [TR] Interact with Sean and Karen in Fix-It during Three Days Left, then in the morning of One Day Left, help them to cook food.
Math Whiz: [TR] Interact with the math tutor poster in Othermart. Go to the house where you have to help Brent with his math work. Make zero mistakes while helping him.
Grammar Whiz: [TR] Same as Math Whiz, but it's a different poster and a different house. You have to help Joy with her grammar work. Make zero mistakes.
Good dog: Pet either Shadow Dog you can find in eastern Vast Forest, or [TR] Hector/Lucas in Faraway.
Bees?: [TR] Find a hive of wasps in Faraway Park. You don't have to win to get the badge.
Reduce, reuse and recycle!: [TR] Complete Fashionable Mom's quest during Two Days Left, and then on One Day Left, go to the Hidden Lake in Faraway Park and you'll find the Recycultist's HQ where you fight Recyclepaths. They are the only people you can battle with in the True Route.
Littering is bad, recycling is better: Go to the menu and to Snacks. Find an unnecessary item such as Tofu, and trash it.
R.I.P: When you're in Black Space, and you go to the Town Area, interact with a specific tombstone. You end up in the Developer Room. Read every other tombstone inside the room to get the badge.
They call me Scarethrow: In the Junkyard, you can come across Mr. Scarethrow, who encourages you to throw projectiles at him, since he makes various sound effects. Have Kel throw projectiles at him approx. 68 times until he repeats his introduction (If he says: "They call me Scarethrow. I am stuck in the ground" after throwing 68 projectiles in a row, you got the badge). While during all of the projectile throwing, he rants about his life and how hard he had it.
The "You think you're clever?" achievement for naming the protagonist Omocat is apparently absent in the console version of Omori.
The [TR] means True Route and [HR] means Hikikomori route.
