This didn't pop for me the first time. I watched your video to make sure I was in the right place. haha, it popped the second time. Thanks for the visual! Thumbs up.

I guess it's super random, as I've made 4 runs thus far and this room hasn't appeared. Also, in agreement with Myckel Jel above, the achievements are somewhat glitchy. The one for getting to the sewer level only popped for me on my second run.

This room was super hard to find for me as well. I had to run around aimlessly a few times and assumed it just didn't spawn for me. I luckily found it while I was trying to finish my 20 escapes.

Not really sure whether I was just missing it the whole time or it didn't spawn but I believe the room is accessible only via a hallway within a hallway if that makes sense. I didn't test it and have already uninstalled it but from the initial hallway after getting the flashlight, I recalled finding this room after following the red wire to the square-shaped room where you can go left or right and still end up going to the same hallway on the other side. In that square-shaped room, there was an unremarkable door that lead into a hallway from which I then found this ex-smoker's room. That same square-shaped room had 2 other doors that I don't remember opening up so maybe that's why everyone's missing this? Thinking 2 of the 3 doors didn't open and not bother with the 3rd.

If anyone can test it and confirm this theory, that'd be cool. Otherwise, good luck to all finding this blasted room.
