A Unity Assist happens when you press the swap button while being attacked (costs three burning gauge segments). An animation of your partner swapping out with you will play confirming that you did it correctly.
You can easily grind this during the chapter "Battle in Oris Plaza" in Luffy's Episode mode where the mechanic is introduced, you are given unlimited Burning Gauge allowing you to do Unity Assists until you have the trophy.
