Complete the ram herding puzzle in the Glen on your first attempt, not hitting the bells to reset at all. (Note: If you reset, you can't get this achievement unless you do another playthrough of the main game!)

If you're like me and tried to solve the puzzle without the video first: instead of hitting the bell to reset, you can fast travel from this area and back to reset the rams but still get the achievement in case you goof it up.
