"Tipping the Scales" gear comes unlocked when you purchase the DLC. Simply spend very few skulls (between 4 and 6 should do it) on one of the DLC weapons or artifacts and this will unlock.
I used the Gatling gun.
If you want to do this quickly, you could probably just do the first level and then you'll be able to do this one. From starting the game to achievement in under 10 minutes.
