The Forever levels can be incredibly tough, and will test your patience and skill. You unlock the Forever levels by completing the game. (Finishing all the Tier 3 zones)
You should take your time, plan ahead, and be sure to manage your mass very effectively. The rest will come down to luck. You must complete three levels in all three of the zones to unlock the last four achievements.
