You must play on either Medium or Large difficulty. And you cannot play with Zen Mode enabled - this will prevent most achievements, particularly all of the "Unusual Way" series, from being awarded.
The shower is a powered appliance which heals you from the alien goo (known as "Dirt"). When you get hurt by goo or by aliens, your develop purple spots. The purple spreads as you take more damage, until you are knocked out. In Solo play, this means game over.
An alternative to the shower is the water bucket. You cannot heal yourself, but you can wash off another player. Unfortunately, this means Solo play is a nightmare as you cannot splash yourself. And if Spiky aliens appear, you WILL take damage trying to get rid of them.
However, there is a possible way to heal and not use the shower, even in Solo. One powered appliance is the Watering Bot. This bot will refill buckets, water pumpkin gardens, splash dirty power sockets, and soak you if you are dirty. If you get lucky enough to find a watering bot (or have unlocked it f and have not used a shower, try your best to complete the match. Give up on other tasks if necessary, as completing with under three points of damage is extremely difficult. Having a shower in later runs can help you recover from a bad situation to complete other attempts.
