This was actually super duper easy. I just ran through everthing on the easiest difficulty and got the Ashael achievement along with it.

I strongly suggest running through chapters on nightmare to get the feel of insane before starting.

Just complete the game without hiding in a barrel or closet but you should get this when you complete it on Insane. If you have followed my mini-Guide, then you won't ever hide in a barrel or closet.
