How to unlock the Thoroughly Baptized achievement in Outlast 2 - Definitive Guide

I got this early in the game after you and your wife are separated. You will meet the leader of the heretics, the lady licks your face , is very blasphemous, and leaves you alone in the woods. Down the path you will come to houses on water with enemies patrolling. As soon as you can see their flashlights and hear them you can submerge in the pond to the left side of the path. I set a timer on my phone and grinded it out there with no danger. Watch for the damage indicators as listed in the solution above.

Evidently it helps when you don't crouch and uncrouch. instead, crouch then use a peek button to go up for air.

The best spot to obtain this is during Chapter 3: Lementations, Sub-Chapter "The Lake".

When you start the level run on down to the lake itself and slowly start to crouch walk into it until you are submerged and stop there! If you go any further the Demon that is in the lake will come after you, but you should be fine on the shore. During this time Blake will start to loose his breath which will be indicated when he starts to make sounds and the screen flashes red so get him out at this point for a few seconds then go back in.

You don't have to wait until you get to the lake later in the game. You can unlock the trophy basically at any pond where you'll find yourself underwater when you're crouching or crawling. Just make sure to surface regularly to catch breath.

You need to spend a total of 10 minutes underwater. This has to be done in a single playthrough. Go to a place with water and go underwater until Blake starts to suffocate. Surface, wait a moment and go underwater again. Keep doing this until the trophy pops but do note that entering a barrel with water in it, doesn't count as you have to go underwater in an actual river or lake. A good place to do this is in 'Genesis - The Fields', since you will eventually come to a large river.
