This one is very simple, simply go through the game without hitting to "Hide" in a closet or barrel. Some hiding is allowed, as it is occasionally mandatory in order to progress. You can crouch down and hide in logs, under beds, or in tall grass at any point.

After being pulled form the school and threatened with a machete, the chase sequence will end once you pass through a door and the wooden beams collapse behind you. Turning a corner, you'll be better off hiding under one of the beds as the enemy breaks down the door and passes you. This is allowed.

In the chapel, after Josiah calls out for Mary and Knoth, you must hide in the confessional on the right wall in order to get a cutscene to progress.

In the school, after answering the tongue phone, you have to run away from Loutermilch and down a hall. Jessica runs into a locker, you have to follow her into it in order to progress.

If you get spotted on lower difficulties, running enemies around is usually more effective than trying to hide somewhere. If you need to hide, hide behind things in the environment as opposed to inside a closet or barrel. If you're on any difficulty but Insane, it might also be worth it to just take a death and retry the area with a more stealthy approach.

I strongly suggest running through chapters on nightmare to get the feel of insane before starting.
