Any item that is blue (rare) or higher (purple and yellow) can have mods installed. If you dismantle these items, you will learn the ability to craft this mod and add it to other equipment. You should dismantle every blue or above item until you unlock this achievement. Mods come in three tiers, and each tier of a mod type counts separately toward this achievement. As mentioned in the description, they have to be unique mods, so dismantling two items with the same mod will only count once.

There are 406 Mods available in the game but you'll need to play through on all 4 Classes to get those. You'll get more than the 125 required for this trophy playing on just one. There are 3 levels of modifications, 1 - blue things, 2 - purple, 3 - gold. Once you collect all level 1 mods, you don’t have to check blue items

The total number of modifications you have unlocked/purchased can be controlled in the Rewards / Actions with Items / Modification Collector section.
