For this achievement you need to either upgrade a green or blue rarity item. It is also worth noting you need to reach a certain point in the story before you unlock the option to craft.
I recommend doing strictly green items as blue items require titanium to upgrade which are very scarce whereas green items require iron for weapons and leather for clothing which you can get from dismantling and harvesting ores (iron).
I saved up a bunch of green items and iron and leather and went and simply improved the rarity of 15 items, it cost me around 300-500 iron for each weapon and the same in leather for clothing items.

Thanks for this guide, didnt realise greens only took leather or iron to upgrade, so makes this cheap as chips towards the end of the game

Fastest, easiest and cheapest way
1. Only improve the green items
2. You will have a lot blue and purple items while you play the game
3. Put some blue items in your inventory box
4. Create a new character, get the blue items from box and sell them, buy the greens and put them back into the box

You probably won't do this that often with higher level gear as new gear drops so often, especially if you play on the highest possible world tiers. So I would suggest hanging on to 15 green (uncommon) rarity items by putting them in your stash as you play through the first few story missions, then you can use those later to quickly knock this achievement out. You'll need to dismantle a fair few items to get what's needed to upgrade the green item rarity, but that won't be a problem with how many you dismantle for mods.
Interact with Zahedi at any camp to do this. You will rescue this NPC as part of the story.
It's highly recommended you get this trophy with Green/Unusual gear. Gear that is Blue/Rare and Purple/Epic will cost you Shards to improve the rating and you'll want to save those for when you actually need them for something outside of a trophy.
