My achievement is glitched at 97% even though I have dismantled nearly 400. Is there a fix?

As mentioned above, any item that does not have a mod should be sold, otherwise you should be dismantling it. This should come fairly quickly as loot is fairly plentiful in this game. You can tell before you dismantle something if you already have the mod or not - you'll see a small square with four dots inside next to the mod on the item if you already know that mod for crafting. If that's the case, feel free to sell the item instead, unless you want crafting materials.
You can Dismantle gear at any time via the inventory menu but you won't be able to actually Mod items until you complete the main story quest in The First City and have rescued Dr. Zahedi. When you have access to him, talk to him and choose "I'd like to Mod...". With the Mod or Inventory menu open, you can select one or multiple items with , then hold to dismantle it/them into parts. You'll need to do this to actually collect Mods for Inventor's Almanac so there's a chance this will most likely unlock naturally.
As a rule of thumb, collecting Mods is far more important than selling items. So before you sell any item, check the Mods the gear has to see if you're missing it/them. If you are, dismantle it so you can collect them. If you already have it/them, you can sell or dismantle the item (up to you and what trophies you still need).
