How to unlock the Adrenaline Junkie achievement in Overwatch 2 - Definitive Guide
I know a lot of people have been struggling with this achievement so here's how I got it.
There are three ways Junker Queen applies wounds:
1) Jagged Blade (Gracie) or
2) Carnage
3) Rampage Ultimate
The tricky part on this I believe comes from applying follow up wounds after the ultimate because of the length of the dash. So find a payload map with a confined space.
Alright how I did this was defending the final point of Route 66. To the right of the payload finish (its marked by a circle on the floor) as you come out spawn are some stacked boxes that separate a flanking route. As the enemy team came on the payload (all five were on it) I ran out of spawn with ultimate ready and triggered Rampage and hit 4. I intentionally aimed at the "wall"/ crates to stop my dash and keep me close proximity to the targets. As soon as I hit I turned around and thumped 2 more with Carnage. Finally I melee'd 1 with the jagged blade as soon as I was able for the seventh wound.

Anyone else whos gotten this, could they also share. Ive done this multiple times in different ways and still no achievement.

In case anyone would like a visual reference for getting this achievement. In slow motion to make for easier viewing.
It might be beneficial to flank the enemy team when trying for this. If they know you're coming you will likely be killed before performing the combo, or stunned out of your Ultimate.

So this achievement has been glitched since release. A few weeks ago I figured out what exactly was bugged about Kiriko’s cute spray and should have realized the same approach could be taken with Junker Queen’s pixel spray. As of the moment Blizzard has only fixed this with Kiriko so players aren’t even aware the issue was tired to character selection.
The objective is to apply 7 wounds at once. The rampage, Carnage, and Gracie each apply a wound. You can spot if a wound is applied by a brief yellow slash over the target (something to look for in replays if you clip your moments). Ideally this means to rampage through a crowd of enemies then immediately swing and stab away, but the present glitch makes this challenging

I posted a video showing several of my failed attempts that actually had more wounds and what I did this morning to unlock Adrenaline Junkie and tips to get this done within a few QP matches. The best method I found to get this achievement is to throw Gracie (Jagged Blade) to an enemy farther back behind enemy lines. Ideally it hits a target for one wound. When you recall it it will hopefully hit another enemy(s) for another wound or two.
Then use your ultimate, Rampage, to dash into the enemy team. You can really only get this achievement if you hit at least three targets with this. These wounds will last the longest.
At this point you're at about five or six. Your ult should put you right in the middle of the enemy team for a quick melee and then use of Carnage. As long as both of those hit it should all add up to seven or more.
A lot of this has to do with where the enemy team decides to put themselves. I got this on Eichenwalde at the top of the hill right at the corner over the top of the bridge. At least at the moment I was able to tell I got the achievement when I got disconnected.
Update: Added the game clip from when I got this.

Usually try to go for this on payload maps. Best advice I have gotten is throw your knife in the ground in front of you before using your ultimate. Line it up and after your ult your jagged blade will return to you and should hit some of the same opponents you just used your ult on. Then follow up with carnage and don’t forget your standard melee creates wounds as well! Make sure the team of at least 4-5 people are grouped up and bait a Kiriko purify before going for this if the enemy team plays her. Good luck!!
