After making it through the hotel with no deaths and no lost limbs.
A cutscene is triggered afterwards you can control Matt again leave the room through the open doorway turn right and crouch under the body.
This will trigger a 2nd cutscene afterwards when you have control over Matt again wait for Doc to put his arm inside the locked elevator trying to grab Matt.
Walk over to Doc and press
to open the more menu and then press 
to take revenge.
After revenge is successfully taken the achievement should pop up shortly.
