Make sure to have all your limbs in order to get this achievement.
After getting through the hotel and regaining control after the cutscene. Walk upto the 2 Doc documents sitting on the desk next to the tub pick them up.
Walk over to the chair pick up a 3rd Doc document sitting on the chair close by the desk and then walk over to the far opposite end of the table and pick up 2 more Doc documents for a total of 5 before leaving the room to get onto the elevator.
After the cutscene and your in control of Matt walk towards the rear wall of the elevator 3 Doc documents will be on left rear wall.
1 Doc document will be on the back rear wall, and the last 2 Doc documents will be on the right rear wall for a total of 11 Doc documents.
Upon successful completion the achievement should appear.
