Once you eat enough pellets, the desired bonus item will appear just under the Ghost Spawn location. Once you eat the bonus item, you must wait until you get a Game Over for the achievement to unlock. Eat this particular item on level 11 or 12 when it spawns to receive this achievement after the game.
Hamket
The Bell is the bonus fruit for stages 11 and 12. You must eat a total of two of them across both stages in one game to earn this. If you get Game Over, you start back at zero fruits eaten.
