How to unlock the Exterminator achievement in Painkiller Hell Damnation - Definitive Guide
Bit glitchy this one. The achievement is based on your "Kills in Survival" stat under "Player Statistics" not your "Leaderboard" score. The games has a habit of losing kills when you exit the game to dashboard so I popped the achievement with 1759 showing on the Leaderboard. As such I suggest doing this in one sitting, but it's not as bad as it sounds.
I was using Stakegun and getting 100 kills every 5 minutes on Train Station. There's a room with a clock over the entrance, with armour, rocket launcher and ammo, health and stakegun ammo. Just camp in here close to the exit and kill as the enemies funnel though (nothing spawns behind you).
Could be a good place to boost
EDIT - given the glitchy tracking of kills in Survival it might be better to boost Extinction in single player.

I couldn't find Dadmix's placement, but the room mentioned in the solution (with the rocket launcher) works pretty well for idling with the painkiller. I placed myself just to the side of the entrance. (You need to be far back enough that the bayonet guys can't hit you and the flamethrower death explosions don't hurt you. I was in between the 1st bench and the potted plant, with the doorway to my left.) Then I threw a piankiller alt-fire across the opening. Not the fastest thing ever, but low effort. Sometimes the enemies forget about you and stop coming, so you may need to go out and taunt them periodically. And sometimes they will get past and kill you. But it's a decent way to make progress while you are doing something else and can check every 10 minutes or so.
Completing a level in single player seemed to save my progress through a dashboard. Enclave is super quick a bunch of damage cards.
