Choose the level "Party Bus" and the chainsaw character. With RB chainsaw a clear view. Look for a bomb in the item case, if it is not there quickly finish and try again. You must finish the level, fail, or fully quit to get a new level layout.
Once you have the bomb, slaughter until 1 victim remains. You can dance to annoy the last one. You need the last victim far to one side. Then quickly place the bomb, upon killing only yourself you get the achievement. Takes some tries but .

Have probably quit out at least 50 times and all i get is the stun gernade and smoke gernade. Am i doing something wrong?

I just kept annoying the last victim by dancing and she killed me. Weird. But it worked.

Septicschizo is totally correct. You will get knocked out a few times before actually dying though.

The last person can die from the bomb with you and still get the achievement.

I can't get this to unlock despite having done it several times on different levels. Any advice?
UPDATE: So ALL the other achievements have unlocked, including the one for unlocking all the achievements, but this one still has not. I don't know what to do.

I deleted the game, reinstalled it, and then deleted all data for the game everywhere. Loaded it up, tried doing it in the tutorial but it didn't work. Went ahead and tried it on the first level and it unlocked.

Nice! I just did the same and this time it worked for me. For some reason I had previously deleted my saved data since I only had the first few levels available but it still had all my characters unlocked. Deleting Save Data again from Everywhere and then hitting the unlock requirements got it to pop. Cheers!!

This achievement unfortunately seems glitched for me. I've lost the level with 1 victim remaining on approximately 5 different levels and it won't pop for me. Wondering if I had a networking hiccup and it didn't unlock correctly but has anyone seen the same thing? It's the only achievement I don't have which doesn't make sense =(

HighT3chR3dn3ck -- The same thing happened to me as well. I tried completely deleting my save both from console and the cloud to start completely fresh but despite dying with 1 person remaining multiple times during my initial play-through and again in later attempts of trying to get this to pop it still doesn't seem to want to trigger for me even when the criteria should be met.
