When attacking an enemy with elemental damage, there is a chance that the enemy will ignite (fire), freeze (ice), or be shocked (lightning). Personally, I had a hatchet that dealt both ice and lightning elemental damage. I only attacked using the skill gem molten strike which added fire damage to my attacks. Eventually all the stars aligned and I killed a monster affected by all three effects at once.
Ultimately, you are looking for some combination of skill gems, support gems, and weapon properties that give your attacks all three types of elemental damage at once. There are a large number of combinations that could work, and depending on your class there may be perks in the passive skill tree that could help you. Just make sure that you are dealing all three elemental damage types at once, and you should eventually pop this achievement.
