Looks like they haven't updated the achievement description for Xbox on this one. If you check your achievements on your Path of Exile profile the description is actually:
"Have three Watchstones socketed in each Region of your Atlas."
So essentially once you get to the end game and start doing maps you'll have a chance to spawn the "influence" of a Conqueror in a region. Once their influence has spawned, after running a few more maps in that region you'll be able to progress their quest line, talk to Kirac and he'll show you where their citadel is; talk to Zana and she'll spawn their citadel for you (you have to complete the map she spawns and kill the boss and then a portal to their citadel will open). Killing the Conqueror will drop a Watchstone*.
So you then have to repeat this until you have 24 watchstones (3x the 8 regions on the map).
*Conquerors can appear in each region multiple times however they can only drop a Watchstone once in each region, so if Drox spawns in Haewark Hamlet and you kill him, you'll get a Golden Watchstone. If you make him spawn in Haewark Hamlet again then, when you kill him, you won't get another watchstone from him. So you have to try and be a bit tactical about where you make them spawn.
Click the left thumbstick to open the watchstone inventory, this is split up by region and the tooltip will tell you how many watchstones you need to have socketed in that region to be able to spawn which conquerors. e.g. if the Golden Watchstone says "Number of socketed Watchstones: 3 required to spawn Conqueror" for a region, but you only have 2 socketed in it, then Drox will never spawn there. You're free to move watchstones around as you please without any penalty though so you have quite a lot of control over the process.
Once you have 20 watchstones the quest will update for you to take on Sirus. Conquerors will no longer spawn while the Sirus portion of the questline is active so, if you're not ready for him, talk to Zana to spawn the portals then just go in and out of all six portals until they're all used and the questline will move back to spawning/killing the Conquerors - though if you're not on a Hardcore league you may as well have a go at him (note before talking to Zana: as far as I'm aware the level of Sirus is determind by the Awakening Level of your Atlas, so if you want the "easiest" possible fight then make sure you don't have any region with 4 watchstones socketed so that your Awakening Level is 0). The only, pretty big, caveat is that as far as I've experienced once he gets to his final phase the frame drops make it impossible to actually fight him. The laser attack he does completely locks up the game and when it unfreezes it's because I'm dead... Fought him 3 times at this point and had the same result every time.

- The Elder
- The Eater of Worlds
