You need one or more remnants of corruptions for this achievement.
With your remnants in your inventory, you want to find a monster trapped in an essence crystal that already has four essences attached to it. Then you want to use one of your remnants (by pressing Y) to corrupt the trapped monster. If you are lucky this will add a fifth essence mod. Releasing and then killing the monster will grant you the achievement. If you are unlucky will need to search for another monster trapped with four essences to use another remnant of corruption on.
There is no guaranteed way to find a trapped monster with four essences; just pot luck.

A data point: rumor is you can only get 4 essence monsters in T7 maps and above. Personally I seemed to get more consistent 3 essences in higher tiers like T12 or T13, where T7-9 I seemed to get more 1-2 essence monsters. Ultimately I got 4 different 4 essence monsters all on T12 or T13 maps and on the 4th monster the corrupt produced the quintessence.

^ I just got my second four-essence monster in Residence, which is only Tier 5 right now. I would imagine, as you suggest, that although unsubstantiated, that the higher tier map you're in, the more likely more essences is (it'd be kind of silly otherwise) but you can definitely get 4-ess mobs at least as low as Tier 5.

Sometimes essence monsters spawn with 4 essences. If you get lucky and that monster isn't already corrupted -> use a remnant of corruption on it and with a lot of luck a 5th essence is added. Kill monster, BAM.
