Story related and cannot be missed. UNLESS: you let your party members die without ressurection, you don't allow a companion to join you, or you dismiss a party member from your service. The final companion is Nok Nok. The quest to obtain him is Mother of Monsters, which is a story quest.
At the beginning of the game, it is important to note that certain companions can die if neglected for too long. Basically meaning you should focus on the main quest, until you've obtained all the companions. There are multiple orders in which to do the msq, but I prefer this for act1: tomb, tree, technic league, Temple of the Elk. This order ensures all of act 1 allies are obtained, but note that Tristian will be injured and unusable until act 2

Linzi, Valerie, Harrim and Jaethal will be decided by your choices at the end of the prologue. You'll need to choose good or evil and chaos or law. Good gets Linzi, Evil gets Jaethal, chaos gets Harrim, and law gets Valerie. Then, you can pick up one of the two you missed at the ancient tomb and another in the kobold camp at old Sycamore.
Tristian is at the Temple of the Elk.
Octavia and Regongar are at the Technic League Encampment.
Jubilost is at the Ford Across Skunk River during Troll trouble. He might refuse to join at first. Talk to him again after defeating the trolls to recruit him. Can definitely be missed.
Ekundayo is at the ruined watchtower during Troll Trouble. Follow the dog.
Nok-Nok is at the goblin village during the season of Bloom.
