This trophy is very time consuming and specific. You may want to dedicate a campaign run just focusing on this trophy. You don't have to. Here is a guide that includes every item and boss you'll need: https://steamcommunity.com/app/640820/discussions/7/16386687...
From what i understand, you need to pick up floor quest items, so that the bosses will drop their boss quest items. If you kill the bosses before you collect the floor quest items, they won't drop the boss quest items. You need 4 boss quest items to unlock the final boss. Be ready for an extremely hard fight (unless you know what you're doing). I heard holy bombs helps but I have no idea. I used an asortment of experimental mercenaries, and had to reload the fight 4 times, on the easiest difficulty. Party level was 12-14
To repeat what i said in a more direct manner: TL;DR: if you kill a boss, and he doesn't drop his boss quest item, you forfeit this trophy. Fret not, you can reload and look for the missing pieces
A personal note: i attempted an iron-man campaign in order to achieve this trophy and "Apocalypse Can Wait," at the same time. Apocalypse does not stack with this trophy. You have to do the dlc version of tenebrous, on ironman mode. Such will surely test your sanity :D
