To unlock this trophy you must play through the Varnhold's lot DLC and get to the point where you see a dog NPC wandering around the puzzle room.
You use a "Break Enchantment" on the doc to transform it into a monster, fight it and kill it.

This is in the Varnhold DLC, during the quest "Forgotten, Forbidden, Forsworn," when you go into the cyclops tomb with all the teleporting and illusions. Make sure you have a character that can cast remove curse or break enchantment. When you get to the big room with the imprisoned Ankou in the center, look for a dog running around. SAVE and prepare for a tough fight. Cast remove curse or break enchantment on the dog, and, if you roll well on your caster level check, you'll get into a fight. If not, reload. You'll get some great loot as well as the achievement for winning this fight.
