How to unlock the Ticking Away achievement in Pathologic 2 - Definitive Guide

This is a story related achievement and is earned after completing the opening sequence, which teaches you the basic controls of the game. Even though you acquire some inventory items during this, you'll constantly lose your items as you embark through the strange dream sequence.

The achievement will unlock after you have finally disembarked from the train and begin Day 1 in the town.
