Difficulty: Overkill or above is recommended
Weapon: Kunai Knife (Melee) - Unlock level 54

Other Equipment: Concussion Grenade or Shuriken - Unlock level 25
Load up a heist on overkill or above and get a police assault to begin. Once it begins, get ready for a big group of enemies to appear, about four or five most likely. Use a flashbang to stun them and then go ahead and stab each of them at least once.
If there are still enemies standing after everyone has gotten stabbed, continue to stab them. There is a chance that the Kunai does not poison enemies, however it appears that chance is quite small.
An alternative method could be to use the Shurikens to inflict poison. On lower difficulties, they cause a lot of damage and will more than likely kill the enemies quite quick. But Kunai paired with Shuriken on Overkill should net you the achievement without much trouble. The bow and arrow (from western DLC)with a poison mod is also an option.

When the cloth door is pulled aside, lure two more guards a little closer and dominate them.
Then just throw your shurikens quickly at three, even if one dies early, as long as you are quick, it counts

I would recommend using the Shurikens as they have been coated in poison which deals damage over time. There is good heist to do this achievement on. It is Watch Dogs, under the Hector contracts and play on Overkill difficulty as you will have more enemies at the start. When the door comes off the van right at the start of the heist, come out and start throwing your Shurikens by pressing 
at everyone outside and with a bit of luck, you will get three enemies poisoned at the same time. If not just restart the heist and try again.

There are quite a few ways to poison an enemy in Payday 2. However, most of them are really powerful weapons and will kill them before you can have three poisoned at the same time. Also getting Poison arrows in a random card drop can take ages for some of us. Luckily the easiest way of getting this trophy is through the shuriken Throwable which everyone will have access to.
The shurikens in Payday 2 are coated in poison and will slowly damage an enemy once they have been hit by one. The poison effect on these last roughly 2 seconds so you need to hit three enemies that are very close together in quick succession in order for them to all be poisoned at the same time. Normal cops will not survive a hit by one of these so you'll need to do this on the Heavy Swat enemies.
Load up either Birth of Sky, Or Slaughterhouse heist and hit three of the swat enemies quickly with your shurikens. Both of these heists have these types of guards at the very start so you will be able to get it done fast. The trophy will be awarded to you once three enemies were poisoned at the same time without dying.
