Just thought I would put a up a solution rather than a guide

Bank heist: Solo (stealth)
Difficulty: Any, as it doesn't change the mechanics of the level during stealth. I did it on One down (I like to live life on the edge).
Guards: Always 4 which is the requirement.

Summary guide: for quick and easy in and out (that's what she said😂 Couldn't resist):
Additional equipment: keycard, Vault key
If your comfortable with the layout of the bank do your thing you know there a 3 roaming guards and a manager that will always be on the move and the 2 spawns for the camera room.
Once done and dispatched using *shurikens* (throwable) proceed into the bank, kill or take personal credit visitors down and then proceed to front office and repeat ensuring to take down the tellers first which is key as there response time for triggering the alarm is either as slow as the getaway helicopter in aftershock or as fast as a late night romp with the missus.
Set up the drill and find the packages,if you can be arsed, to pass the time.
Steal the items once drill has complete.

Please note:
Using a saw to open ATMs and Deposit boxes does not void the achievement so feel free to rinse the place dry if that is your forte.

There are plenty of guides if needed to stealth a bank, haven't tried it in coop but happy to help if I'm available.

Yeah a lot of achievements can glitch out I have had 8 glitch on me, I found a way to get them to pop if all Else fails and I tried everything before finding this, if your try all the basics such as system reset, hard reset, delete cache, delete saves and then resync from cloud, delete game and saves redownload and sync from cloud and none of them work Your save is fully corrupted like mine for those achievements but some may still pop just not the ones you know are glitched.. a work around is back your save up externally, start and brand new save (literally from scratch) and do the achievements you can even delete your console save, go offline, do the requirements and then log back on and it should pop, give me a message if you need help as some achievements may require you to at least be levelled e.g overkill without dying (one that glitched for me) I don't mind helping

Play any bank heist on normal, and make sure you don't use anything else than shuriken.

Keep in mind that a lot of these achievements can be glitched and wont unlock even if you meet the requirements.

First, you need to equip the Shuriken as it is a throwable weapon. Next, equip upgraded Jammers (which are in the Ghost Skill Tree) to stop anyone from calling for backup and cable ties as you only get ten Shurikens. There are a few good heists to do this on and it is best to do them on normal as you will not need many bags of loot. The first is the Jewelry Store from the Bain contracts, but you will need to kill a lot of civilians. There could be two or three guards around or in the store, and there may also be two cops at the end of the street.

If there aren’t any cops at the end of the street then you will have to restart the heist.

If there are cops at the end of the street you are good to go. Kill the cops (you will not need to answer pagers with these two cops) then kill all the civilians you see and those coming down any of the streets in the area. Now you want to go around the back of the store and take out any guards but you will need to answer their pagers. After that, it is best to pick the lock on one of the windows to get into the store. Then, plant a Jammer and start killing or getting civilians down and tie them up (there may be one more guard in the store). After you have sorted everyone you can get the loot and make your escape.

NOTE: You must kill the civilians with the Shurikens, not your gun.

Another good heist is Shadow Raid also from the Bain contracts on normal as you won’t need many bags of loot.

Plus, Art Gallery also from the Bain contracts just keep in mind the paintings and guards may not be in the same place. This video just shows it can be done.

The Shuriken is essentially a throwing star and can be equipped as a throwable weapon in Payday 2. These will kill a normal level guard with one hit so it's best to use a stealth mission for this. Depending on your ability to stealth through a mission this will be a easy trophy. I suggest you use this Stealth Build by b33croft during your attempts at this trophy as it'll keep you from being spotted. I've outlined a few methods below on how you can go about getting this trophy.

Method One - This approach is going to be for more experienced players at Payday and will require you to be able to stealth through the normal Bank heist contracted to you by Bain. Make sure you have the stealth build set up from above and have your Shuriken equipped as a throwable in your inventory. If you're having trouble stealthing this heist you should refer to this HOW TO STEALTH - PAYDAY 2 - (Bank Heist - How to Solo Stealth DW) by b33croft to get an idea of how to stealth this heist.

There will be four guards that will be on duty at the bank. 3 patrolling the Bank and 1 in the camera room. It's best to get the Vault on the side without the bank tellers on it so you can avoid that whole side of the bank and just drill open the safe. If you're Solo there are many Civilians around and tying them all up isn't really an option. If they get alerted you'll need to kill them with your shurikens too. For this trophy you need to have a body count of at least 4 while only using the shurikens. If you use any other weapon it will void the trophy. Once you complete the heist with the met requirements you will be awarded the trophy.

Method Two - This is by far the easier way of going about getting this trophy. First off Buy the Shadow Raid contract from Bain on the Normal difficulty. The reason why you want it on the normal difficulty setting is that you will only need to secure 3 pieces of loot. That should only take a minute or two if you know this heist. You also are going to need a full stealth build with upgraded ECM jammers. Link for the build is above.

Once you have the loot secured and the Escape is available you want to place your ECM jammers down and sprint into the yard of the Murky water warehouse and start throwing Shurikens at the low-level cops without armor on. Once you've killed at least four of them the trophy will pop. Below is a video of this being done thanks to GaminGuys

The Ukrainian Case robbery is fine on difficulty.
We kill 3 guards and crack the safe and take the tiara for Vlad. At the moment of escape, we kill one civilian with a shuriken and the achievement is yours.
