How to unlock the Salem Asylum achievement in PAYDAY 2: CRIMEWAVE EDITION - Definitive Guide
Difficulty: Mayhem
Solo or Co-op: Both
Sadly, this achievement had glitched on me when I was playing, but I certainly did reach the train at 4:43.
UPDATE: I have unlocked the achievement through the roundabout way of going offline. The only difference in gameplay I had while playing offline was that I finished the heist to ensure that it would register. Maybe it's worth a shot for you to do so as well.
This method was presented to the TA forums by amacteur. It was revolutionary for everyone with glitched achievements and is clearly still effective! For those who do not know about this offline method, here are the steps to do so and hopefully trigger your achievement:
Press the Xbox button to open the guide.
Select Settings.
Select All Settings.
Select Network.
Select Network settings, and then select Go offline.
Launch the game.
Go to Crimenet.offline (yep, no choice!)
Select the job you need for a specific achievement and meet the requirements.
Finish the job.
Once you get back to the game menu, press Xbox button and quit/close the game.
Press the Xbox button to open the guide.
Select Settings.
Select All Settings.
Select Network.
Select Network settings, and then select Go online.
Launch the game.
The achievement may not pop on screen but will be "unlocked" (see after).
Once you're in the game menu, press the Xbox button to open the guide, go to your achievements, click on Payday 2, and TADAM! your achievement is unlocked!
The icing on the cake for me in terms of speed was unlocking the basic tier of partners in crime. This increases your speed by 10% whenever you have a converted enemy. If you incorporate that with the ghost tree's speed bonuses, you should be quite fast. Inspire will certainly help if you are playing co-op as it increases your teammate's speed by 20% for 10 seconds if you shout at them.
For those curious about the wheel landing on "KEY," it is possible, however very unlikely. If you get "KEY" on the first or second spin, you are golden. It cuts about 1:30s of your time.
If you are playing solo, take a look at the video below as it shows a path that will allow for you to get through the prison relatively quick. The only thing that should give you trouble should be the second location of the wheel. It is not always the same location as the video. If it is on the opposite side of the map, you'll want to get a good path down and try to aim to be at the second wheel by 2:15-2:20. The wheel takes about 1 minute and 30 seconds should you not be interrupted by an enemy stopping it. If you arrive at the second wheel between 2:15 and 2:20, this should give you about 55-50s to activate the escape and run straight to the trains; Take a look at the video for a solid path. You should have a relatively smooth run to the train unless a taser comes out, in which case, kill any taser. They will ruin your run unless you are in the middle of spinning the wheel. Anyways, soloing this achievement is certainly harder than playing with a co-op partner.
If you are playing co-op, the host should be the person in charge of getting the keycards. Considering they will have no latency, it is essential for speed. After gaining the Keycard from the first wheel, host player will run to the bridge to use the keycard and player 2 will wait here:

It is right in front of the first wheel's location and is a shortcut inside of the prison. This can save valuable time for your team if player 2 can activate the wheel quickly. After that, the host will run to place the keycard for escape with player 2 right behind them ready to activate the escape. Then everyone books it to the train. Note: Only players that reach the train car in 4:44 will be the ones to get the achievement. Not the entire team.

Thankfully you can get this and All Cloakers Eve (completion of level on Death Wish difficulty) with ease using the method shown in the video. When you teleport in the traincar, run to the opposite side of where the C4 is to be placed all the way to the end and this will trigger the achievement to pop. You will need to play the heist on Mayhem, Death Wish, or Death Sentence difficulty. Mayhem should be, with a doubt, your target difficulty for this one. The heist needn't be completed in order to achieve this trophy. However, if playing co-op, only those who make it to the train within the time constraint will receive the trophy.
Unlike the other speed run counterparts in the game, this is a rather tight time limit. To have any chance at this trophy, you will only want to spin the two wheels required to bring the escape activation up. If you spin a third wheel, it will be impossible to achieve the time limit. If an enemy gets close enough to the wheel to stop the 60 second countdown, the n you will have to restart. You will want to rush past as many enemies as possible when going to each new area. If playing co-op, use good coordination to split up and use shortcuts to get to the second wheel faster. Be sure to get to the escape trigger together in co-op, as you all need to be in a zone to activate the zipline. Map knowledge is key to making it to the zipline without wasting time getting lost in the prison. Despite the very narrow time crunch, you can use handy trick to make this easier.
If playing co-op, you can duplicate the first keycard. To do this, you must have two players prepared to start spamming the button as the wheels time gets towards 00. Duplication happens if both players happen to press the button at the time the card becomes available for pickup. Typically, you would need to communicate a time to each accurately hit simultaneously. However, this card is easier to dupe because it is only available after the wheels timer hits 00. You might not dupe a card every time but it is easy to do within an attempt or two. Below is a video showing the card duplication, as well as splitting up to cut more time off the mission.
After the first roulette has been placed, one player takes a place near the wooden door (opposite the roulette)
The host takes the card and runs to the grate - as soon as he opens it, the wooden door will open. The player who stood near it gets the opportunity to quickly get to one of two places for installing the second roulette (there are only two places - near and far, they appear randomly). But even if you get a distant place, still run. Place the roulette wheel, defend it, and 15 seconds before the end, the host runs into the room where you need to insert the card. The second player takes a card and quickly leaves the gaming session. The card ends up with the host, thereby saving 10 seconds. I (in the described situation) ended up in the carriage at 4:35. If the tape measure had been in the closest place the second time, it would have saved another 5 seconds.
It is important to get into the carriage no later than 4:43 - i.e. At 4:43 you should already be running along the carriage. At 4:44 the trophy will no longer be given.
The trophy drops immediately - there is no need to complete the mission.
