When playing the very first heist, No Rest For The Wicked, just put on the mask immediately and the achievement is yours.

Just waiting for a patch, last resort would be wiping all game data. I’m going to wait and see if they’re adding more achievements before doing that. I hope they don’t

Once you load into the match, you need to manually put on your mask my holding cn_LB. If the mask gets forced on later in the match, the achievement won't unlock.

The trophy only drops if you put on the mask yourself by holding , otherwise, if the game itself put it on you, the trophy will not fall out.

Also, it doesn’t work in training, you need to wear it in the game.
