A Cool Clear is when you have cleared the entire board of pegs, apart from one blue and one orange peg. The text of the achievement is a little misleading as the last blue peg will always turn into a purple hi-score peg - it still counts. Bounce your ball off the orange peg to hit the purple to complete the level to unlock this achievement.
This is easiest done in quick play mode on any of the early levels, simply pick your favourite. Try to leave a blue peg close to an orange, then proceed to carefully clear the rest of the board, focusing on getting free balls from the bucket or with high scores.
As the achievement description says, this only unlocks when you're playing a single player game mode - online or offline multiplayer doesn't work.

I would like to add that you can't get this in challenge mode or by using a green peg's 'power' to ensure that you hit the orange then the blue (I've done this with a zen ball and a fire ball and neither worked),

Would like to also add it doesn't have to be a blue pug, i just got this achievement i cleared the board (1-1) but had 3 orange pegs left (so no blue, only 3 orange) achievement popped! get in!

Go onto Quickplay and leave an orange and blue peg near each other. You need to make a pretty good shot that hits the last orange first and then hits the last blue in fever mode.

In order to get a Cool Clear you must hit the last orange peg and then hit the last non-orange peg. I recommend using Bjorn on the 1st level and using the special ability when you're about to hit the final orange so you can make sure your shot will rebound onto the last non-orange to earn the "Cool Clear" trickshot!
