Have the rapscallion trait. Attend the autopsy, hide when Werner comes. Step out to confront and say apologize, then say you called him sawbones that can get a man punched then you have the option to headbutt. Achievement unlocks. I didn't get this the first time I headbutted him. Not sure exactly what the issue was but I see two possibilities.
First is that I had restarted playing the game from Quick Resume which sometimes causes an issue, but I had done that multiple times before with Pentiment without achievement issues.
The second thing I suspect it may have been is that I had the big heads setting all the way up. Not sure why that might cause an issue as I'd think the achievement would just be triggered by selecting the dialogue option.
Whatever the issue was I restarted the game, loaded a previous save, turned heads back to normal and tried again and it unlocked that time.
So if anyone else has an issue with unlocking this one, restart your game and make sure heads are set to normal size and try again to see if that fixes the issue.
One final note: if the game has saved after missing this or other achievements, you can load older saves though that isn't immediately obvious. You do it from the main menu by unintuitively selecting New Game, moving the cursor to the save slot of the current game if needed, then hitting Y to bring up older saves in that slot.
