It would take up far too much space to write a full guide for all the levels so I will sum it up.
You have to complete every level of the co-op agent campaign on agent difficulty (easy). Grab a friend or host a match on xbox live You can check which levels you have done on the level select menu.

Just wanted to point out that this can be done locally with a second controller solo by yourself. Some of the boss fight are a bit hard with two controllers, but completely do-able.

Refer to "Completed Single Agent" for more information on the Agent difficulty. This can be done locally or online. Complete all 14 missions in co-op, on Agent (or a higher difficulty) to unlock this achievement.
