This can be completed with the first hidden object puzzle that you come across in the Antechamber, at the start of the game.
After some dialogue with Darius, a hidden object puzzle will become accessible just behind him. You will need to find 13 drops of water to complete the puzzle, and if you do this with less than 4 mistakes, the achievement is yours. This can easily be done here, just be very careful that your cursor is centered over each water drop to make sure you select it on the first attempt. If your cursor isn't centered over the water drop, it may not count it, and it will count as a mistake.
Also, be advised that 3 of the water drops are behind objects that will need to be moved first (leaves, broken stone pieces, and rocks) so they are not in plain sight. Simply select the areas that I have double-circled in red and yellow on the solution below to move the object out of the way, and then select the water drop underneath. :)

Finishing an HOP with fewer than four mistakes is very easy in this game, as I found them much easier than other similar games. However, there aren't very many in this game compared to others, so don't delay too long on this.
You have a few options for this:
- Take your time and make sure you're certain about each object before you click on it.
- Find and click on all objects except one, making as many mistakes as you want. Locate the final object but don't click on it. Quit out of the game and reload it, and you should be back at the start of the HOP. Now click on each object without mistakes.
- Use the walkthroughs in the "Expert Gamer" achievement solution. There are multiple HOPs that aren't randomized (the ones that require a set number of a single item, or show pictures of the items you need to find) so you can use screenshots or a video to find all the items without any mistakes.
