How to unlock the Extracurricular Excellence achievement in Persona 3 Reload - Definitive Guide
2) Go to Tartarus after earning this achievement
3) Occasionally, Elizabeth will let you know that there is a missing person in Tartarus
4) After entering Tartarus, your team will tell you exactly which floor to find the missing person
5) Go to the floor the missing person can be located
6) Interact with missing person

On June 18th, Elizabeth will call you on your cell phone and ask you to come see her. When you do (which is automatic, by the way, and not optional), she’ll tell you about some missing people who have stumbled into Tartarus and need rescuing.
The first of the three missing persons (that can be rescued before the third Full Moon event) will be Ayako Yoshimoto, who can be found between floors “48-53” (specifically 50F) before the deadline of July 6th. So, make your way into Tartarus and make your way to the 50th floor, where you’ll find Ayako. As soon as you escort her to the entrance, the ‘Extracurricular Excellence’ achievement will unlock.
A few things to note here: firstly, I’d wait until after June 26th, and that way you can rescue all 3 missing persons at once (Kenjiro Tsutsumi & Yoshimi Miyama too), all of which need to be saved before July 6th. And secondly, don’t forget to go to the police station after to collect your reward.

You need to save someone to receive a letter of gratitude; you will need it before Christmas to receive an item on Elizabeth's behalf. The reward for rescue is given by a policeman from whom you can buy equipment.
