When all the enemies are down, you can hit the A button to perform an All-Out Attack. That said, if the person you're attacking with has a status ailment, they won’t be able to initiate an All-Out Attack. On top of that, if another party member has a status ailment, but the character who downs the last character does not, you can still perform an attack without them, but it will do less damage than a full strength All-Out Attack, obviously.
Simply perform 50 All-Out Attacks and the ‘Making the Dream Work’ chievo will unlock.
Edit: Can confirm, it finally unlocked for me on my first all out attack during new game plus.

The key to success in all Persona games is to perform All-Out Attacks. You can perform All-Out Attacks once you've downed all the enemies you're facing off against - you down an enemy by either triggering their weakness or landing a critical attack.
