Every time you take its health bar down it switches through ALL of the tarot cards eg. Fool, Death, Hanged Man etc.
Then a cut scene, then a very simple "on rails" fight between you & the Nyx heart, very straight forward. Once you do the final move the achievement will pop during the cut scene.

Story-related, missable.
In order to make it to the good ending, and unlocking both this and From Shadows into Light (40G), you need to make the correct choice on December 31st. That choice is, of course, to choose to save Ryoji, and not kill him. The only way to face Nyx, and thus seal it, is to make it to the top of Tartarus for January 31st, and that’s only possible if you’re a pacifist.
Then, when you’re at the top of Tartarus, after having beaten the proper endgame boss, Nyx Avatar, you’ll face off against Nyx itself in a scripted boss battle.
After a few rounds, and after the main character is energised, you’ll be able to cast the Persona ability, Great Seal, which will not only unlock ‘The Great Seal’ achievement, but will also bring to a close Persona 3 Reload’s main story - well, except the epilogue.
